I'm really, really happy with the job AppRiver does filtering spam and viruses. You basically set them up as your SMTP proxy (they get all your mail and forward it) and they do the filtering for you. AppRiver's accuracy rate is in the very high 90's for spam, and it's almost completely hands-off on the administration side. They're also what I would consider very inexpensive, even for small businesses.
Standard disclaimer - I'm merely a satisfied customer and have no stake in the company. I'd just suggest folks look at both hosted and installed application options.
You're not the first one to mention them. I've worked with clients that have been very happy with AppRiver, and the same can be said for FrontBridge. I've heard very good things about both services.
